Narrative:
Written off (destroyed) July 30, 1972 when ran out of fuel and make a wheels up force landing into a corn field at Sully, Jasper County, Iowa (at approximate co ordinates: 41°34′43″N 92°50′44″W). Of the five persons on board (pilot and four passengers) the pilot and two passengers were seriously injured, and the other two passengers were slightly injured. According to the following excerpt from the official NTSB report into the accident:

"PROBABLE CAUSE(S)
PILOT IN COMMAND - MISMANAGEMENT OF FUEL - INADEQUATE PREFLIGHT PREPARATION AND/OR PLANNING - MISCALCULATED FUEL CONSUMPTION
MISCELLANEOUS ACTS,CONDITIONS - FUEL EXHAUSTION - INTENTIONAL WHEELS-UP FORCED LANDING
COMPLETE POWER LOSS - COMPLETE ENGINE FAILURE/FLAMEOUT- BOTH ENGINES
EMERGENCY CIRCUMSTANCES - FORCED LANDING OFF AIRPORT ON LAND"
