Narrative:
PROBABLE CAUSE:THE ACCIDENT WAS INADEQUATE MAINTENANCE OF THE HELICOPTER, ALLOWING A TAIL ROTOR DRIVE SHAFT BEARING TO EXCEED WEAR LIMITS. THIS RESULTED IN HIGH LOADS ON THE TAIL ROTOR DRIVE SHAFT, AND THE RESULTING FATIGUE FAILURE OF THE DRIVE SHAFT.
