PROBABLE CAUSE:THE FAILURE OF THE PILOT TO MAINTAIN CONTROL OF THE AIRPLANE WITH AN OPEN DOOR. FACTORS RELATED TO THE ACCIDENT WAS THE FAILURE OF THE PILOT TO MAINTAIN AIRSPEED AND THE DOOR OPENING AT LIFTOFF.
