Narrative:
On July 14, 2011, at 15:30 local, the privately-operated Lake LA-4-200 was departing Wellman Lake, Manitoba, on a local flight. During the takeoff run the aircraft porpoised and lifted off in ground effect, then stalled, struck the water, and sank in shallow water near the shore. The three occupants exited the aircraft and made their way to shore without injuries.

The aircraft sustained substantial damage - presumably so much that it was later written off (as damaged beyond economic repair); the registration C-GKND was cancelled on December 7, 2011.
