Probable Cause(s) and/or Contributing Factors
- Left Main Gear Number Two Tire Burst. (inside left).
- Delayed decision to start the abort procedure due to unidentified vibration caused by a tire blowout.
- Improper handling of the emergency brakes by a crew member not qualified as a flight engineer, resulting in loss of ground control and runway excursion.
