Narrative:
163267 (MSN 701222) w/o May 19, 1993 near Quantico, VA during inspection flight after undergoing maintenance. Four Marines killed.

It is reported that "A Department of Defense investigation blamed the crash on pins that were installed improperly by Marine Corps personnel who maintained the helicopters, resulting in engines flaming out as the pilot attempted an "autorotation" maneuver during the test flight."
