Le 11 septembre 2003, Transport Canada a été avisé que le 14 septembre 2000,l'hélicoptère BELL 206L4 immatriculé CGLBH, avait subi des dommages au cours d'un vol d'entraînement aux mesures d'urgence à l'aéroport de Lachute. La poutre de queue aurait été endommagée au cours d'une auto-rotation; manoeuvre permettant de se poser en cas de panne moteur. Le BST n'avait pas été avisé. Transport Canada, section maintenance, en fût avisé le 7 mars 2001 par le rapport annuel sur la navigabilité aérienne. La lettre de l'inspecteur, envoyée le 25 mai 2001 au propriétaire de l'appareil, lui demandant des informations sur l'étendue des dommages et le certificat de conformité concernant les réparations est resté sans réponse jusqu'au 11 septembre 2003 où l'inspecteur a obtenu les documents réclamés le 25 mai 2001.
