Narrative:
Update: Follow-up information received from Airworthiness ? West [2015-08-24]: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The CASI contacted the operator for more information and was provided with additional details on this occurrence. The CASI was advised that operations appear to have been normal at the time of the incident. The engine is undergoing tests. The CASI was satisfied with the operator?s information and maintenance actions and advised that no further Transport Canada follow-up action was required at this time. , 2015-07-28 - JRCC SARSUM Trenton Report#[T2015-01471]: (425700N 0813798W - Strathroy, ON). Detect only 406 signal received on Goes East and West. Investigation revealed that a Zimmer Air Services Inc Robinson R44 II (C-FRAP) crashed in a corn field while conducting spray operations. No injuries or requirement for Rescue services., 2015-08-06 - UPDATE: TSB Report#A15O0118: C-FRAP, a Robinson R44 helicopter, was being used to spray fungicide on fields of corn. The pilot approached and overflew one field checking for hazards and wind conditions, and then began a spray swath across the field. Midway through the first swath, the pilot heard the low rotor horn sound. Manually rolling the throttle up did not prevent the rotor RPM from continuing to drop. The pilot then attempted a quick stop and a controlled autorotation into the corn. The helicopter was substantially damaged by the impact but the pilot who was wearing a four point shoulder harness and a helmet was not injured.
