Narrative:KA-6D Intruder BuNo 151824/'NE-523' of VA-115 on board the USS Midway: Lost on training mission on August 20 1975. The carrier was off NAS Cubi Point in the Philippines at the time. Both crew - Commander Denny Weeks (pilot) and Lt G. T. Wyckoff (Bombardier/Navigator) - ejected. A portion of the B/N's seat mechanism structurally failed when the catapult fired on a night launch from the USS Midway. The failure initiated the ejection sequence for the B/N, who found himself pushing off the side of the ship as it went by. Flash from ejection probably disoriented the pilot severely.
