Narrative:
On June 22, 1999 at 11:15 EDT (Eastern Daylight Time = 17:13 local time) a Robinson R22 helicopter, N220WP, was substantially damaged during a mid-air collision with a Zlin Z-42M airplane registration G-EWOA near Grossenhain, Saxony, Germany. The flight inspector and student pilot of the Zlin airplane, and the pilot and passenger of the R22 helicopter were fatally injured (a total of four fatalities). Visual meteorological conditions prevailed and the type of flight plan filed was unknown.

According to German aviation authorities, the helicopter departed Kueritz, Germany, at 07:58, destined for Dresden, Germany. The helicopter made one stop en route, on a farm, near Kueritz. The airplane departed Riesa airfield, 20 miles northwest of Dresden, at 10:24, for a VFR check-flight.

The helicopter and the airplane collided at 2,000 feet amsl near Grossenhain, Saxony, Germany, 14 nautical miles north of Dresden. Both aircraft were in level flight. The helicopter had a heading of 150 degrees and the airplane had a heading of 240 degrees.

As a result of the collision, the airplane lost its tail plane, and the helicopter lost its landing skids. Both aircraft impacted a field and were destroyed. Following the impact with the ground, the Zlin airplane caught fire and completely burned.

The weather conditions at the accident site were: wind from 310 degrees at 15 knots, gusts to 25 knots; visibility 20 km up to 50 km; a broken layer of cumulus clouds at 3,600 feet agl, and a few cumulus clouds at 2,500 feet agl with light rain showers.
