38 when engine failed after take-off, aircraft stalled & dived into the ground at RAF Habbaniya, Al-Anbar Province, Iraq.

According to the official Air Ministry announcement in "Flight" magazine (July 7 1938 page 21 - see link #6):
FLYING ACCIDENTS
P/O Charles Alexander Stephen and Aircraftman 1st Class Harold Davies lost their lives in an accident which occurred at Habbaniya on June 29 to an aircraft of No. 30 (Bomber) Sqn., Habbaniya, Iraq. P/O Stephen was the pilot and Aircraftman Davies was the other member of the crew."
