Narrative:Written off (destroyed) December 4 1964 due to a CFIT (Controlled Flight Into Terrain) when collided with trees on final approach to Drake Field, Fayetteville, Arkansas. Aircraft was engaged in an "air taxi" operation. Of the five persons on board (pilot and four passengers) the pilot and three passengers were killed, the sole surviving passenger was seriously injured.
According to the official NTSB report into the accident: "...pilot did not have approach plate or map-maneuvring over dark hilly terrain after IFR let down". "Pilot fatigue" was also attributed as a possible cause of the accident.
