Narrative:
Written off (destroyed) February 14, 1969 when crashed at/near Goodwin Field Airport, El Dorado, Union County, Arkansas (at approximate Coordinates 33°13′16″N 092°48′48″W): aircraft undershot, collided with trees and crashed during night landing approach. Aircraft on controlled VFR flight plan, although the flight conditions were IFR weather.

The pilot (the sole person on board) was killed. According to the official NTSB report into the accident: "PROBABLE CAUSE(S) - PILOT IN COMMAND - MISJUDGED DISTANCE AND ALTITUDE. FACTOR(S) - WEAT HER - LOW CEILING - RAIN - FOG"
