Narrative:According to local authorities in northern Afghanistan, a NATO-led International Security Assistance Force helicopter crashed in northern Sar-e-Pul province.
At least one pilot of the helicopter was injured during the incident. According to the Army mishap review the crew experienced loss of tail rotor thrust when the aft hanger bearing sheared at the #5 tail rotor shaft. During touchdown in the resultant forced landing the a/c rolled onto its left side sustaining Class "A" damage. The mishap was traced to improper maintenance during re-installation of the t/r drive shaft when the work was split over separate day and night shifts resulting in inadequate torque being applied.
