Narrative:
Anson 6086, (ex-RAF R9892) of No. 1 BGS (Bombing & Gunnery School), RCAF Jarvis, was on a night bombing training flight. There were three occupants on board including a RAAF pilot and two RAFVR air bomber students. The crew had completed their mission over Lake Erie and were returning to the airfield at Jarvis when witnesses saw the aircraft drop a flare, perhaps in an attempt to illuminate the ground for a forced landing. Before they could make a landing, the aircraft crashed to the ground and exploded, killing the three crew.

Initially, the suspected cause was an airborne fire that caused the left-hand wing to explode. The AIB determined that there was fire in the air, but the wing did not explode. It was believed, due to poor maintenance, the left-hand prop was incorrectly installed and caused severe vibrations. This caused a fuel line to become loose and start a fire in the left-hand engine and nacelle, leading to the crash.
