The pilot and a passenger on board the helicopter departed from Pietersburg Civil Airport on a private VFR flight to Tzaneen Aerodrome where the helicopter is kept. However, en route to Tzaneen, it was established the helicopter impacted the grass covered terrain in an open area with only a few scattered trees in the area at the outskirts of Moabi Village in the Limpopo Province.
The accident occurred at approximately 1130Z, 25km (14nm) east of Pietersburg Civil Aerodrome.
The helicopter was extensively damaged on impact with the terrain and evidence indicated that the engine was under power during the impact sequence and the outer section of a main rotor blade that was located approximately 115m from the main wreckage.
The passenger sustained fatal injuries during the impact sequence. The pilot sustained serious injuries and succumbed of his injuries in hospital after approximately two weeks.
