Narrative:
Substantially damaged at 21:59 HST (Hawaiian Standard Time) January 30 2013 when involved in a hard landing at Kona International Airport, Kailua-Kona, Hawaii. According to the official NRSB report into the accident:

"The pilot was practising an auto rotation when the main rotor rpm decayed. The flight instructor took control of the helicopter and attempted to correct the low main rotor rpm. However, the helicopter landed hard in a nose-high attitude, which resulted in structural damage to the tail boom. The pilot reported no pre-impact mechanical malfunctions or failures with the helicopter that would have precluded normal operation.

The National Transportation Safety Board determines the probable cause(s) of this accident as follows: The pilot’s failure to maintain adequate main rotor rpm during a practice auto rotation and the flight instructor's delayed action, which resulted in a hard landing."
