A Zivko Edge 540T impacted airport terrain while maneuvering over the runway during a practice session. The wreckage came to the rest in the river. Pilot Steven Andelin and two ground spectators were fatally injured.
PROBABLE CAUSE:
- The late application of flight controls and engine throttle to maintain the desired flight level.
- Momentary loss of situational awareness or loss of consciousness involving depth sense coupled with overconfidence.
- The decision to excessively decrease the flight level at minimum distance above the surface, abandoning operational safety during the activity.
