Causes/contributing factors
The cause of the incident is considered to be the failure to extend the landing gear due to the instructor's lack of adherence to the ATO's procedures and those of the aircraft itself.
Contributing factors are considered to be:
- Lack of oversight/approval of flight schedules by the Fly By school's chief trainer to ensure correct post briefing briefing times.
- The instructor does not recognize himself as being unable to perform the instructional flight.
