Narrative:
The Cessna A188 agricultural aircraft with registration HK-1368 proceeded from the Neiva York runway, located at kilometer 46 on the Villavicencio – Puerto López route, to carry out the second spraying flight over a 30-hectare banana crop.

During the spraying, while maneuvering to align with the field for a pass, the aircraft collided with a tree, losing control and crashing into the ground in an inverted position. The aircraft came to a stop with substantial damage, and no fire occurred.

The pilot, the sole occupant of the aircraft, suffered fatal injuries.

The investigation determined that the accident occurred due to the following Probable Causes:
- Collision of the left wing of the aircraft with a natural obstacle (tree) located at the beginning of a banana crop where the spraying work would take place. The impact caused the aircraft to uncontrollably crash into the ground in an inverted position.
- Excessive confidence of the pilot, dismissing the need to perform a flyover of the plot and its surroundings to identify possible threats that could affect the safety of the operation maneuvers.

As a Contributing Factor, it was found:
- Inadequate standardization of procedures and risk analysis by the organization's Operational Safety System, as they did not conduct a topographic survey of the plots and crops where spraying would be performed to identify possible threats that could affect the normal development of the operation.
