The Cessna R172K was scheduled to perform a flight between the city of Villavicencio (SKVV) and Caño Colorado (SKCJ), Guainía, in order to transport a nurse. The aircraft took off from Villavicencio's Vanguardia airport at 07:10.
While flying at an altitude of 7,500 feet, and after approximately 02:45 hours of flight, the aircraft experienced loss of power and engine shutdown. The pilot executed the restart procedure, as established in the POH, without obtaining satisfactory results.
In the meantime, the pilot made an emergency call and made a 360° turn trying to locate a river to ditch or land in its vicinity, without finding it. Therefore, he performed a tree landing, a maneuver in which, due to the effect of the impacts against the trees, the aircraft ended up inverted on the ground, with substantial damage.
The pilot suffered minor injuries. The passenger suffered a major injury, consisting of an arm fracture, without exposure. And they applied basic survival techniques and to facilitate their location.
That same night, the two survivors were rescued by a Colombian Air Force helicopter which transported them to Mitú.
Probable cause
Fuel depletion and engine shutdown, as a consequence of inadequate flight planning and deficient pre-flight inspection by Dispatch and the pilot, who did not foresee and did not supply the aircraft with the necessary amount of fuel for the flight.
Contributing Factors
Organizational deficiencies in the company consisting of the absence of effective dispatch procedures to plan and execute flights so that they are performed within safety margins.
Lack of a procedure in the Operator to guarantee the fuel supply with the appropriate amount of fuel according to the distance to be flown and the flight times.
Deficiency on the part of the Operator and the crew, by not having a Navigation Plan for the flight, which would determine, among other aspects, the intermediate check points en route, the calculations of distance, time and consumption of each leg, the amount of fuel remaining at each point, and the emergency or alternate aerodromes en route.
Deficient navigation procedures on the part of the Pilot, by not verifying fuel consumption and the amount of fuel remaining along the route, not realizing that the fuel was insufficient to reach the destination, and therefore not returning to the aerodrome of origin or proceeding to an emergency aerodrome en route to make a safe landing.
