Narrative:
The helicopter was engaged in heli-logging support at Nahatlatch Lake when the accident occurred. It was in the process of setting down approx 750 lbs of choker chains with a long line, when it departed without releasing the load. The aircraft was then seen to aggresively flare (pitch nose up) and thel oad was released. The longline became caught in the rotor system and the aircraft went out of control and crashed into the ground. The pilot sustained critcal injuries and was flown (via S61) to the Chiliwack hospital. The helicopter was substantially damage and was consumed by a post crash fire.

June 23 Revison from TSB Summary: THE BELL 206 HELICOPTER (C-FBQG) WAS MANOUVERING ABOVE A STEEP HILL SIDE IN SUPPORT OF A HELI-LOGGING OPERATION, WHEN THE CHOKERS ATTACHED TO THE END OF THE EXTERNAL LOAD LONGLINE BECAME SNAGGED ON A LARGE LOG. WITNESSES REPORTED THAT THE HELICOPTER REACHED EXTREME PITCH ATTITUDES BEFORE THE CHOKERS RELEASED. THE LONGLINE SUBSEQUENTLY SNAGGED ANOTHER LOG. THE PILOT APPEARED TO LOSE CONTROL OF THE HELICOPTER AND EXPERIENCED DYNAMIC ROLL OVER WHILE ATTACHED TO THE GROUND BY THE LONGLINE/EXTERNAL LOAD. THE HELICOPTER STRUCK THE GROUND, ROLLED OVER, AND CAME TO REST, HELD UP BY THE LONG LINE, ON THE HILL SIDE . GROUND CREWS REACHED THE AIRCRAFT IN TIME TO TEMPORARILY PUT OUT A POST-CRASH FIRE WITH THE AIRCRAFT FIRE EXTINGUISHER, HOWEVER A SECOND FIRE ENSUED. WITHOUT OTHER APPROPRIATE FIRE FIGHTING EQUIPMENT, THE GROUND CREW WERE FORCED TO QUICKLY EXTRICATE THE TRAPPED PILOT. THE FIRE CONSUMED THE AIRCRAFT, BUT WAS EXTINGUISHED BY A COMPANY HELICOPTER (S61) WHICH LATER TRANSPORTED THE INJURED PILOT TO THE HOPE, B.C. HOSPITAL. THE PILOT, WHO WAS WEARING A HELMET, RECEIVED SERIOUS INJURIES TO HIS RIGHT ARM AND TORSO. HIS HELMET DISPLAYED EXTENSIVE IMPACT DAMAGE AND LIKELY PREVENTED SERIOUS HEAD INJURIES. PRELIMINARY INFORMATION REVEALED NO MECHANICAL FAILURE OF THE HELICOPTER PRIOR TO IMPACT.
