During the water landing, the float-equipped aeroplane tipped forward and stopped with its engine in the water.
Contributing factors
The following factors may have contributed to the failure of the inter-float mast support during the water landing:
- the failure of the float manufacturer's maintenance programme to take account of an operation such as that of this ATO, characterised in particular by a large number of water landing cycles;
- the failure to detect the propagation of cracks from bores inside the masts during pre-flight or routine maintenance inspections.
The following factors may have contributed to the continuation of the instruction session until the floats were separated
- the instructor's difficulty in interpreting the trajectory deviation that occurred during the previous landing as anything other than a pilot error on the part of the trainee;
- the instructor's failure to detect and the pilot's failure to report the vibrations he noticed during the subsequent take-off.
