The sightseeing helicopter crashed into an orchard, killing the pilot and both passengers. Initial news coverage wrongly reported that there were four people on board.
Analysis of the helicopter's Lycoming IO-540 engine revealed that residues worn away from polytetrafluoroethylene material in the fuel pipeline had blocked certain fuel nozzles in the engine, which would cause partial loss of engine power. The Robinson R44 probably stalled when the pilot attempted to return to land upon recognizing the engine problem.
