Narrative:THE PLT LOST CONTROL OF THE ACFT WHEN IT ROTATED EARLY DUE TO THE ELEVATOR TRIM TAB BEING SET AT FOUR NOTCHES NOSE UP. THE ACFT THEN STALLED & LANDED HARD COLLAPSING THE LANDING GEAR. THE PLT FAILED TO USE A CHECKLIST.
