Narrative:
B-52D s/n 56‑0595 call sign "Red 2" of 4133 BW(P) (Aircraft from 454th BW). Crashed and destroyed on July 7 1967 in a mid-air collision with B-52 s/n 56-0627 over South China Sea, 20 miles offshore, off Binh Dinh Province, South Vietnam, while changing formation lead.

Crew: From 454th BW, three killed, three survived
Pilot/Commander:Capt George Westbrook USAF survived.
Co-pilot:Capt Harold Dean Thompson USAF survived.
Nav: Capt Charles Herman Blankenship USAF killed.
Rad/Nav: 1st/Lt George Emerson Jones USAF killed.
