Crashed on rotor track and balance flight.
Potentially dur to failure of upper swivelling upper boost actuator due to a production defect and of the Longitudinal Cyclic Trim Actuator (LTCA) clutch

#OTD in 1987: a RAF Chinook [ZA721] crashes near Mount Pleasant base (Falkland Islands), all 7 aboard die. Helicopter was on a test flight after servicing, went down shortly after takeoff.
