El día Jueves 02 de marzo de 2006, fue localizado siniestrado a las 07:45 HLV, por comisión del SAR-AMAZONAS, el helicóptero EV-8440, AS-61D, a 10 millas náuticas (18 Kmts.) en el radial 020° de Puerto Ayacucho (SVPA), Coordenadas: LATITUD 05° 46' 50.5'' NORTE / LONGITUD 067° 32' 48.5'' OESTE, Amazonas, Venezuela, solo se rescato a una (01) persona sobreviviente los otros siete (07) fallecieron.
La persona sobreviviente es la Dra. ISBELIS JAIDID VILLEGAS ZULETA, C.I. N°: V-15.586.413, FN: 01OCT82, EDAD: 24 AÑOS, la misma resulto herida y se encuentra recibiendo atención médica en el Hospital de Puerto Ayacucho, donde ha evolucionado favorablemente.
OCUPANTES DEL HELICÓPTERO: Teniente Coronel del Ejército, JESUS MANUEL RODRIGUEZ SALAZAR, el Capitán JOAN SILVIO FLORES (PILOTO), el Capitán WILLY SALVATIERRA BRITO (COPILOTO), el Sargento Técnico de Segunda RAÚL BARRÁEZ CASTILLO, el Sargento Segundo ERICSON RAFAEL MARÍN, las doctoras ISBELIS JAIDID VILLEGAS ZULETA y MILEDYS CAMPOS, además del señor GERMÁN GARCÍA.
En la tarde del día miércoles 01 de marzo de 2006 un helicóptero militar perteneciente al Ejército Venezolano fue declarado en fase "DETRESFA" al no llegar a su destino.
La aeronave matricula EV-8440, AS-61D, "SIKORSKY", con ocho (08) personas a bordo había descolado de CHAPARRALITO, Estado Apure, Venezuela con destino PUERTO AYACUCHO (SVPA), Estado Amazonas, Venezuela, su ultimo reporte lo realizo a las 17:15 HLV (21:15 UTC) a 20 millas náuticas en el RADIAL 030° de Puerto Ayacucho estimado su arribo para la 17:27 HLV, no llegando a destino.
En la aeronave aparte de su tripulación viajaban dos (02) médicos, (01) un indígena y un (01) oficial superior.
